Boating Incident Sparks Safety Investigation in Virgin Islands

The Marine Safety Investigation and Reporting Authority (MSIRA) has confirmed a significant boating incident that took place on Sunday, July 6, 2025. A Lagoon 46 sailing catamaran, named MV Nohea Holokai, struck a reef shortly after leaving the Anegada Channel, leading to structural damage and partial sinking. Fortunately, all 14 passengers aboard were unharmed and safely evacuated from the vessel.
Investigation Underway to Enhance Maritime Safety
Chief Marine Safety Accident Investigator Mr. Jerome Padmore announced that a thorough safety investigation began immediately following the incident. The investigation aims to gather and analyze relevant information to clarify the circumstances surrounding the accident. The MSIRA has stated that the findings will be compiled into a safety accident report, which will be made public once the investigation concludes.
The primary objective of this investigation is to enhance maritime safety and prevent similar incidents in the future. It is important to note that the investigation will not assign blame or determine liability. Instead, the focus will be on identifying safety issues and making recommendations for improvement. The MSIRA has committed to keeping the public informed with updates as the investigation progresses.
Additionally, the MSIRA reminds the public that all marine accidents must be reported to the authority, as mandated by Section 7 of the Merchant Shipping (Accident Reporting and Investigation) Regulations, 2020. This regulation underscores the importance of transparency and accountability in maritime operations within the territorial waters of The Virgin Islands.
The Marine Safety Investigation and Reporting Authority operates under the Premier’s Office and is tasked with the independent investigation of marine accidents. Guided by the Merchant Shipping regulations and the International Maritime Organisation’s Casualty Investigation Code, the MSIRA aims to improve maritime safety through fact-based investigations and proactive recommendations.
